Dr. Keating, you are my favorite eye doctor, how do I follow you?

ALL patients have the right to choose their doctor(s). My current patients have the option of transferring their care to my new office. Desai Eye Clinic is currently scheduling appointments (for both current and new patients) to see me beginning January 1, 2026. In an effort to make this transition as seamless as possible, multiple options for scheduling appointments are offered:
- Patients may call Desai Eye Care at 337-235-6263 to schedule appointments.
- Patients may use the digital scheduling form found at https://eyedesai.com/schedule/ to request an appointment. Look for the “Requested Physician” dropdown selection and pick Dr. Patrick Keating.
- Patients can use their phone camera to scan this QR code to be taken to the digital scheduling form to request an appointment.
Dr. Keating, where will your new practice be located?
Desai Eye Care is located near Lafayette General on Hospital Drive. We are located on the 3rd floor of the professional office building. The address is 155 Hospital Drive Suite 300, Lafayette, LA 70503.
Dr. Keating, what insurances will be accepted at your new practice?
Desai Eye Care accepts most MEDICAL insurances, including commerical plans and Medicare. We do not accept primary LA Medicaid (but LA Medicaid is accepted as a supplemental insurance). Because the services performed at Desai Eye Care are medical in nature, we do not accept vision plans. However, patients with vision plans are able to use their medical insurance for an examination and still use their vision allowance toward glasses or contact lenses.
Dr. Keating, will you still be performing surgery, if so where?
Yes, I will continue to offer full surgical options to my patients including laser cataract surgery, premium IOLs, PanOptix IOLs, Toric IOLs, glaucoma treatment, LASIK, PRK, astigmatism correction, presbyopia correction, minor clinic procedures, dry eye procedures, and more.
I will operate and have surgical privileges at both Lafayette Surgical Specialty Hospital (1101 Kaliste Saloom Rd., Lafayette, LA) and Lafayette General Oil Plaza Surgical Center (1000 W. Pinhook Rd., Lafayette, LA).
Dr. Keating, how can I get my medical records forwarded?
Click [HERE] to access our medical release form. It is a digital fillable PDF form that can be filled out directly on your phone, tablet, or computer and then emailed. Alternatively, you can print the form and then scan and email or fax the competed copy. This form should be sent to Azar Eye Clinic and will give authorization to send your old medical records to my new practice, Desai Eye Care.

Dr. Keating, what do I do if I have a problem or need an urgent exam?
I will not start at Desai Eye Care until January 1, 2026. If you need my services before that time, please contact the Azar Eye Clinic at 337-235-7791. After January 1, 2026 I will no longer be associated with Azar Eye Clinic.
